Rotted subfloor in upstairs bathroom

We are completely remodeling our upstairs bathroom following the disovery of mold damage (and because we want to..:)). We removed all of the sheetrock and flooring from the room and have discovered that the subfloor in the area where the tub and the sink used to reside is completely rotted through. I have cut the plywood about one inch from where it slides under the wall and removed the worst board (one ply at a time, ultimately) and discovered that it was infested with carpenter ants. At this point I have three questions. 1) Is using a sawzall and cutting about 1 inch from the base of the wall the correct/best way of removing the subfloor that goes under the wall? 2) Should I trim the leftover subfloor flat against the wall before installing new ply wood 3) Should I replace all of the subfloor in the bathroom to alleviate the possibility of additional carpenter ants?
